Who Is Eligible for Generations Hospice?

A patient who has:

  • Opted for a palliative plan of care, (palliative care is care that focuses on alleviating pain and symptoms rather then seeking or providing curative measures).
  • A life limiting illness with a life expectancy of 6 months or less if the disease process runs its normal course. However, a patient can continue to receive hospice services beyond 6 months if a physician recertifies the illness remains life limiting.

What Types of Diagnosis’s Qualify for Generations Care?

The following are some of the diagnoses which may qualify for admission to Generations:

  • End-Stage Cardiac Diseases: (CHF, CAD)
  • End-Stage Neurological Diseases: (Parkinson’s, MS, Alzheimer’s, ALS)
  • End-Stage Cerebrovascular Disease
  • Metastatic Malignancies
  • End-Stage Renal Disease
  • End-Stage Debility/Failure to Thrive/Multiple Organ System Failure
  • End-Stage Respiratory Diseases (Emphysema, COPD)
  • Acquired Immune Deficiency Syndrome (AIDS)

Generations will work with the patient, family and attending physician to ensure a smooth transition onto the hospice program. Our representatives will meet with patients and families to explain the admissions process and ensure the patient is hospice appropriate and qualifies for Generations services.
